Athletics
At North Side Athletics, our mission is to cultivate a community of student-athletes grounded in discipline, integrity and teamwork. We are committed to creating an environment where every athlete can grow—both on and off the field—while proudly representing North Side. Through purposeful training and consistent support, we develop not only athletic skill, but also responsibility, resilience and sportsmanship. We intentionally provide opportunities and guidance to help student-athletes prepare for post–high school pathways, including college athletics and scholarship opportunities. Our goal is to empower athletes to succeed in their pursuits and make a positive impact in their communities. Together, we build a legacy of excellence where discipline drives our actions, integrity defines our character and teamwork fuels our success.
IHSAA Physicals: The IHSAA By-Laws establishes April 1 as the date for eligibility for new physicals. This means, all athletes wishing to participate in the 2025-26 school year must have a physical completed on or after April 1, 2025.
All parents and student athletes have to create an account on Final Forms. Once parents and athletes create their account, the only requirement is to follow all the steps on Final Forms and upload the IHSAA physical. Once accounts have been created, parents will access their child’s Final Forms account and verify and update information.
- Parents and Athletes create an account and complete all the steps
- Parent uploads the IHSAA Physicians Examination page to Final Forms
- Athlete completes all required forms via Final Forms
